How to Apply:

1. Complete and sign this application.
2. Upload 1-2 optional letters of recommendation.
3. Obtain your parents' or legal guardian's signature, permitting you to participate (if you are under 18).
4. Attach an official or unofficial copy of your transcript or other school-issued verification of your cumulative grade point average (e.g. report card that shows your cumulative GPA).

If you answered no, then you are not eligible to serve on the Mayor's Youth Advisory Council. All applicants must reside or go to school within the Huntsville city limits.

The responses to the following questions will be used to ensure diversity of the applicant pool.

Please choose two (2) questions below to respond to thoroughly (minimum of 250 words, maximum of two pages, double-spaced).

1. Describe 1-2 issues that you believe is/are important for the Huntsville community to address.

• What do you think is an appropriate role for the city’s youth to play in addressing this/these issue(s)?
• How, as a member of the Huntsville Mayor’s Youth Advisory Council (HMYAC), would you work with area youth, local government and local nonprofit agencies to fulfill this role and address this/these issue(s)?

2. What comes to mind first when you think of civic engagement? How do you think youth in Huntsville can be more civically engaged in the community?

3. What does leadership mean to you?

• What unique skills and/or perspectives can you bring to Huntsville's Mayor Youth Advisory Council?
• Why should we choose you to represent HMYAC?
• How would you benefit from participating in HMYAC, and what goals would you have?

Meeting Availability: Council meetings are generally held twice a month on the 2nd and 4th Thursdays of each month beginning at 5:30 p.m. Members are highly encouraged to attend all meetings. Meeting with two (2) or more unexcused absences OR three (3) or more excused absences per semester will be subject to a membership review by the Executive Board supervised by City staff. If you have a recurring conflict with these meeting times, meeting the attendance requirements to remain a member will be difficult.

Photo Consent: I hereby give permission for my photo to be taken during Mayor's Youth Advisory Council activites for promotional or marketing purposes.

You understand that any false or incomplete section of this application may result in the rejection of the application as a whole.

As the parent/legal guardian of the applicant, I hereby grant my permission to the applicant to participate in the Huntsville Mayor’s Youth Advisory Council (HMYAC), which includes, but is not limited to, bimonthly meetings, volunteer outings, day and overnight trips and other activities as they arise. I understand that if accepted, the applicant will need reliable transportation to events and activities.
